Scheduling of new appointments are booked 2-3 weeks in advance. Scheduling is done by appointment only.
If you are a healthcare provider, financial advisor, county, or non-profit organization, and have a
patient/client referral you may send us an email, call us, or fax your referral to us. Referral / intake forms
are available for you and your staff, or you can download one by clicking here.
You may scan your referral form and attach to an email your referral request along with any
additional information you would like to provide regarding your patient.

If you're low on cash, you can still help our organization raise money by simply using your computer. There are two
websites designed specifically to help non-profit organizations raise money by using their web portals to either search
the web or shop online.
The first site is called Goodsearch. It's a place where you can search for anything on the web, just like you would go to
Yahoo or Google to search, only you will perform your search from the Goodsearch site. Our organization gets a donation for

The second site is called Goodshop. They are an online marketplace that donates a percentage
of your online purchases to our organization. You can choose from hundreds of popular merchants
and the experience of shopping through GoodShop is exactly the same as going to the retailer's websites

Guardian Wings of Care (GWC) is now registered with the Kroger Company as a participating non-profit organization in their Neighbor to Neighbor Donation
program.
You can help by printing out the letter at this link http://www.krogerneighbortoneighbor.com/pdf/10000084343.pdf.
Then take the letter with you the next time you shop at Kroger. When you check out at the register, have the cashier scan your Kroger Plus Card and then ask
them to scan the barcode on the letter. This will link your Kroger Plus Card to the Guardian Wings of Care account and a portion of your future purchases
will be donated to Guardian Wings of Care. You only have to take the letter with you once to have the barcode scanned ... your Kroger Plus Card will remain
linked to GWC for the entire Neighbor to Neighbor Donation program year of July 1, 2009 to April 30, 2010.
